ABSTRACT

Objective:To evaluate the efficacy,recurrent risk factors and transferable ratio of treatments with 3 different regiments on children with systematic myasthenia gravis (MG).Methods:The data of 104 children with ocular MG from June 2010 to March 2014 were collected from Department of Pediatric Neurology of Xiangya Hospital and they were retrospectively studied.The patients were divided into 3 groups:a methylprednisolone group (n=44),a prednisone group (n=48) and a bromine pyridostigmine group (n=12).Evaluative system from American MG foundation was used to evaluate the efficacy of treatment and the ratio of ocular MG transformed into systematic MG.Results:The efficacy in the methylprednisolone group was better than that in the prednisone group,and both of them were better than that in the bromine pyridostigmine group (both P<0.05).Methylprednisolone,prednisone combined with bromine pyridostigmine could reach a better long-term efficacy in children with ocular MG.Early treatment with glucocorticoid could reduce clinical relapse.Conclusion:A treatment with high-dose methylprednisolone pulse can improve early clinical remission in children with ocular MG.However,there is a similar efficacy in the long run of different glucocorticoid therapeutic regiments.A relatively order onset age,infection and thyroid dysfunction are recurrent risk factors in children with ocular MG.

ABSTRACT

Gitelman syndrome is a rare disease.It is easy to be misdiagnosed and missed diagnosis due to the diverse clinical symptoms.A girl with long-term hypokalemia,who presented with intermittent pain of lower limb muscle and physical retardation,was treated in Xiangya Hospital,Central South University.Laboratory examination confirmed the severe hypokalemia and metabolic alkalosis.Gene sequencing indicated SLC12A3 gene mutation and the patient was finally diagnosed as Gitelman syndrome.Patients with chronic hypokalemia and metabolic alkalosis need to conduct gene sequencing to confirm the diagnosis.Gene therapy is expected to be the most effective treatment for this disease.

ABSTRACT

Objective To study the coping strategies of patients with mood disorders and explore the corresponding nursing strategies.Methods A survey,using the coping strategies questionnaire,was conducted among 32 patients with unipolar disorder and 55 patients with bipolar disorder including 34 with current depressive episode,14 with current manic episode and 7 with mixed episode.Results As for coping strategies,no difference was found between unipolar depression and bipolar disorder with current depressive episode or mixed episode(P>0.05);while significant difference was found between bipolar depression and bipolar mania on problem solving(t=-2.869,P=0.006),help seeking(t=-2.443,P=0.018),self accusation(t=2.461,P=0.018)and withdrawing(t=2.870,P=0.006).The patients with unipolar disorder,bipolar depressive or mixed episode tended to use mixed coping strategies,with the percentages of 50.00%,41.18%and 42.86%respectively and 64.29%of the patients with bipolar mania tended to use mature coping strategies.Conclusions Coping strategies of patients with mood disorder are associated with the status of the diseases but independent of the categories of the diseases.Therefore,the corresponding nursing strategies include setting aside the problems,encouraging family member to make decisions and conducting active guidance.

ABSTRACT

Objective To study the expression regulation of GATA-3 on Th2 cytokines secretions in rats lung infected by respiratory syncytial virus (RSV).Methods 20 SD rates were random divided into two groups: Control group (10 rats) and RSV-infected group(10 rats, the model was established by RSV intravasation).The airway responses were observed after seven week.Lung tissue of SD rats were stained with HE, pathologic examinations of lung tissue- stained were made.GATA-3 expressions in lung tissue were detected by western-blots, respectively.Interleukin-4 (IL-4) and interleukin-5 (IL-5) in lung homogenate were detected by ELISA.Results Lung biopsy from RSV-infected group showed typical inflammation under light microscope.Airway resistance in RSV- infected group was higher than control group.Compared with control group( 1.231 ±0.31 ), the expression of GATA-3 significantly increased in RSV-infected group (1.743 ±0.21)( t =15.427, P <0.01).The levels of IL-4[(142.3 ± 12.3)ng/L] and IL-5[(167.5 ±14.2) ng/L] increased in RSV-infected group [(69.2±11.6) ng/L, ( 86.6 ± 14.3 ) ng/L] ( P<0.01 ).There was a positive correlation between protein levels of GATA-3 and IL-4 in lung tissues (r=0.864, P<0.01 ), and a positive correlation between GATA-3 and IL-5 in lung tissues(r=-0.756, P<0.01).Conclusion RSV-infection may up regulate the expression of GATA-3 in lung, which contributed to inflammation of Th2 type.
